Employee Enroll/Change/Term Form
Can be used by any existing United Healthcare group to enroll and terminate members, and make any information changes.
Navigate PCP Election Form
When going with the Navigate network, this is the form the employees can use to elect their primary care physician.
Provider Search Flyer
Instructions to find providers. Make sure you select the correct network. Most will be the choice plus network. Some will be the navigate network. Check with us if you are unsure.
Life Beneficiary Form
When electing a life insurance policy, have the employees fill this out to designate who the beneficiary will be if benefits are paid out.

With Virtual Visits, employees and their covered family members can see and speak to a doctor 24 hours a day, 7 days a week using a mobile device or computer, wherever they are. If needed, a prescription can be sent to their local pharmacy. No appointment is necessary–and usually takes less than 20 minutes.
Healthiest You
HealthiestYou is an app that allows your employees to talk to a doctor from anywhere, anytime — with no copay or cost to them. This means they can manage many of the situations that would normally involve a visit to the doctor’s office without leaving home. HealthiestYou also offers a price-comparison tool that may save them up to 85 percent on prescriptions, often beating their copay. The app allows comparison of prices for procedures and research doctors. It’s also a one-stop shop to view medical plan deductibles in real time
